Waikato DHB are asking that photocopied forms - green on one side and white on the other - are NOT USED.
They are unsure where they are being distributed from, but ask if you have them please destroy them.
These green forms were made for surveillance testing in Alert Level 4 at supermarkets, and as such had pre-filled location details. This means the lab does not know where the sample has been taken from and is unsure where to send results to.

The form is pale yellow which provides a visual indication to laboratory staff triaging requests as they arrive that this is a priority sample.
We've been provided with a black and white copy of the form (PDF). If using this black and white version then a yellow ‘PRIORITY HIS SAMPLE’ sticker should be attached to the outside of the sample bag to indicate priority.
To complete the request form please provide the following information.
GP practices that do not have this form can use their own practice generated standard forms which ensures the lab receives location information and knows where to send the report to.
The DHB also require symptomatic/asymptomatic information and if it is for one of your team it must clearly state that this is a swab collector so they can add the correct surveillance code. Please do not just say healthcare worker (HCW) as this is insufficient to prompt required laboratory registration for stats.
