The School Based Health Services team has created wallet-sized cards for school leavers to take to their general practice in order to support the transition of their health records from their school health clinic.
Waikato practices may soon begin to see these brightly designed cards with instructions inside telling the practice how to contact the School Based Health Services team to transfer these notes.
