Offering community-based support for children aged up to 16 years old, the new Tui Ora community paediatric nurse, Rhonda Tito, can help whānau dealing with a variety of health challenges.
This new role has been created as part of the Kahu Taurima initiative and the new service gives priority to those aged under five years old and those who are Māori or Pasifika, with a primary care focus providing nonacute community based care. Rhonda can provide support for a variety of issues, including:
The service aims to improve child and whānau wellbeing, prevent repeated school absences and enforced time off work for whānau, and the associated stress on families, by reducing the need for repeated hospital admissions.
Rhonda works collaboratively with allied health services, providing support for whānau navigating and accessing care pathways.
GPs and other health professionals can refer via this referral form. Whānau can also reach out directly for support by calling 0800 TUI ORA.

By providing community care in the home, I can help manage health issues more effectively, keeping children out of hospital and at school, making important social connections and maintaining their learning which contributes a great deal to their wellbeing.
