Co-response team (CRT) redirect

Overview

This service is designed to support a co-response team (CRT) where mental health clinicians, kaimahi Māori, Police and St John will work together when responding to mental health emergency calls.

From time to time there may be a requirement for primary care to provide support for this service, for mild to moderate patients, as identified by the CRT.

When the CRT is supporting a patient who fits the threshold, the clinician in the CRT will make contact with the general practice where the person is enrolled and seek to confirm if a same or next day appointment is available. If no appointments are available, the CRT may also choose to contact an urgent care provider. 

The CRT will confirm the available appointment and determine with the clinician the most appropriate means of sending through any relevant patient details, assessments and management plan. 

This person is likely to require:

  • PRN medication i.e. something for anxiety, sleep or to start antidepressant
  • referral to HIPs and health coaches and primary health services
  • referrals for physical health needs routine testing etc.

The consultation fee will be funded for eligible patients.

CRT will not redirect a patient unless the receiving practice has accepted them. Patients will be provided with information regarding the appointment that has been made for them.

For conditions that meet Primary Options Acute Care criteria, practices can initiate a separate referral and claim via the usual Primary Options Acute Care process.

Which practices can claim for this service?

All Pinnacle practices in Taranaki region can claim for this service.

Who is eligible for this service?
  • Practices can only claim a redirection subsidy for patients who have been redirected from the co-response team (CRT).
  • Patients who reside within Te Whatu Ora Taranaki boundaries.
  • Patients eligible for New Zealand funded healthcare services.
Who is excluded from the service?

Patients who have not been actively referred from CRT.

When is the service complete?

The episode of care ends when the outcome and claim are submitted.

Payment will be made once an outcome has been lodged for the case.

Claiming guidelines

All ED redirect initial consults should be claimed for by selecting the ED redirect GP/NP consultation.

If the patient meets POAC criteria, please initiate a separate referral for any invoices claimed under that POAC pathway.

CRT redirect GP/NP consultation: $85.00  

This invoice funds a consultation for the redirected patient.

CRT redirect GP/NP consultation after hours: $95.00  

This invoice funds the initial consultation for the redirected patient after 5pm on a weekday, on weekends or public holidays.

Once the patient is at the practice, the GP/NP will assess the patient and determine whether the condition meets any Primary Options Acute Care criteria or is considered normal GP business/non-acute.       

If the patient meets Primary Options Acute Care criteria, please claim one of the above invoices to cover the cost of their initial consultation and continue to claim as you normally would for a Primary Options Acute Care case.

What level of clinical notes do I need to submit?

Practices are required to provide sufficiently detailed consultation notes to determine appropriate use of funding.

Does the patient have to pay?

There is no co-payment to be charged to the patient.

How is the service funded?

The service is funded by Te Whatu Ora.
