Emergency housing programme

Waikato | Preventative | Wellbeing | Under 18 | Over 18 | Over 65

This programme is a collaboration between Te Runanga o Kirikiriroa (Facilitation Service) and general practices.

Funding is specifically targeted to people who are in emergency or transitional housing or homeless within inner city Hamilton and are engaged with Te Runanga o Kirikiriroa. The aim is to re/engage these people with general practice when they are unwell rather than them presenting to the emergency department. The secondary aim is for this population to then enrol with a practice.

Te Runanga o Kirikiriroa will employ a facilitator to work with social agencies and their clients. If the client needs a GP appointment, the facilitator works with the client and the practice to arrange an appointment.

Which practices can claim for this service?

Any practice in the Hamilton City area that has agreed to partner with Te Runanga o Kirikiriroa (Facilitation Service), can offer and claim for this service.

Practices wishing to participate in this programme can contact either the practice system support team or the primary options team.

Service components

Funding is available for GP/NP consultatio, follow up and prescription renewal. All appointments will be pre-arranged by the Te Runanga o Kirikiriroa facilitator.

Patients funded under this service will not be charged a prescription fee for any publicly funded medications, provided the prescription is annotated with the code TKMP

 

Who is eligible for the service?

Patients engaged with Te Runanga o Kirikiriroa facilitator who have had an appointment pre-arranged

Who is excluded from this service?

Unenrolled patients self-presenting to general practice.

Please note if a claim is made for a patient not pre-approved by the Te Runanga o Kirikiriroa facilitator, it will be declined.

The prices listed below are GST inclusive. 

Emergency Housing Programme: Initial GP/NP consultation (unenrolled) (one only): $148.35

Emergency Housing Programme: Initial GP/NP consultation (enrolled) (one only): $92.71

Emergency Housing Programme: Follow-up (unenrolled) (two only): $74.75

Emergency Housing Programme: Follow-up (enrolled) (two only): $57.50

Note follow up cannot be claimed for patients aged under 14 years.

Emergency Housing Programme: Renewal of prescription $23

 

These consultations cover all time and consumables and there can be no extra charge to the patient.

Submit your referral via Primary Options using the initial condition coding: Emergency Housing Programme

When invoicing for the service please lodge an ‘outcome and invoice’ form and select the appropriate invoice(s).

If the patient meets POAC criteria, please initiate a separate referral for any invoices claimed under that POAC pathway.

What level of clinical notes do I need to submit?

Practices are required to provide sufficiently detailed consultation notes to determine appropriate use of funding.

Can a claim for Primary Options for Acute Care be made if the patient presents acutely unwell?

Yes, please claim as usual for cases where a patient presents acutely unwell, and the treatment provided in general practice is preventing an admission to the emergency department.

If the patient meets POAC criteria, please initiate a separate referral for any invoices claimed under that POAC pathway.

Does the patient have to pay?

No. A patient co-payment cannot be charged.

When is the service complete?

The episode of care ends when the outcome and claim are submitted.

Payment will be made once an outcome has been lodged for the case.

How is the service funded?

The service is funded by Te Whatu Ora.
