Funded enrolment consultation

Overview

A funded initial consult is available for eligible unenrolled Taranaki patients wanting to enrol with a Pinnacle general practice.

Please be aware that as a specific amount of funding has been allocated, the service will cease when that funding is exhausted.

Your agreement for Pinnacle funded services applies to this service. By claiming for this service, you have indicated that you have read and agreed to the business rules set out here.

Details

Which practices can claim for this service?

Any Pinnacle practice in the Taranaki region can offer and claim for this service, provided the patient meets eligibility criteria.

Who is eligible for the service?

Patients based in Te Whatu Ora Taranaki area.

Patients must be:

  • un-enrolled or enrolled in a practice outside the Taranaki region
  • AND be over the age of 14 years
  • AND be a current CSC holder 
  • AND be Māori/Pacific OR be pregnant.
Who is excluded from the service?
  • Patients not based in Te Whatu Ora Taranaki area.
  • Patients already enrolled in a Taranaki general practice. 
  • Patients who do not hold a current CSC.
  • Patients who have accessed the funded consult within the last 12 months (x1 funded consult per annum).
  • Patients who have been enrolled with a Taranaki practice in the last 12 months whose enrolment has expired.
Service components
  • This service provides funding for an initial GP/NP and nurse consultation for a patient enrolling at the practice.
  • No co-payment is to be charged to the patient.

It is expected that this consultation will occur within a 6 week period from enrolment.

When is the service complete?
  • This is an inclusive one-off payment for all time spent with clinicians and supplying consumables.
  • Payment will be made once the submitted claim is processed.

It is expected that this consultation will occur within a 6 week period from enrolment.

Please submit your claim within a month of the funded consultation.

Claiming guidelines

The price below is GST inclusive.

Funded initial consultation: $55.00

A 15 minute GP/NP consultation and a 15 minute nurse consultation

This funding covers all time and consumables and there can be no extra charge to the patient.

There can be only one claim per patient per annum. 

It is expected that this consultation will occur within a 6 week period from enrolment.

Please submit your claim within a month of the funded consultation.

Please note if a review of claiming indicates that a claim has been made for an ineligible patient then you will be required to repay the claim.

Please complete the manual template (also linked below) and email it to claims@pinnacle.health.nz.

Please select “Funded Enrolment Consult Taranaki” and submit your claim in the usual way

 

What level of clinical notes do I need to submit?

As cases may be audited practices are required to provide sufficiently detailed consultation notes to determine appropriate use of funding.

Please note if a review of claiming indicates that a claim has been made for an ineligible patient then you will be required to repay the claim.

Does the patient have to pay?
  • No co-payment can be charged.
  • Any further follow up can be charged to the patient but please make sure the patient is aware of this. 
How is the service funded?

The service is funded by Te Whatu Ora Taranaki.  

Please be aware that as a specific amount of funding has been allocated, the service will cease when that funding is exhausted.
