Overview

Best Start kōwae are a suite of best practice based assessment modules designed to improve quality care throughout the pregnancy and postpartum journey. They are designed to support consistent, comprehensive, and quality care by promoting enquiry, management, and referral to support services. The modules are designed with a pro-equity approach for Māori hapū māmā and pēpī Māori. 

The form to complete the modules is embedded into MedTech Evolution, MedTech32, My Practice and Indici PMS.

Which practices can claim for this service?

Any Pinnacle practice that installs the Best Start pregnancy kōwae.

Service components

Wellbeing, equity, and child health are key government priorities. Improving equity for Māori hapū māmā and pēpī Māori is part of the core-design of the Best Start kōwae. 

The point of difference for Best Start pregnancy is the built-in decision support, enabling identification of issues and early intervention, which ultimately will improve health and wellbeing outcomes for hāpū māmā and pēpī.  

There are three touch-points over the completion of four assessments.

  1. Best Start pregnancy: A comprehensive early pregnancy assessment carried out at the confirmation of pregnancy consultation. Meets the requirements for Section 88 non-LMC first trimester claim.​ 
  2. Best Start 2nd trimester check: Recall assessment at 16-weeks to have flu and pertussis vaccinations. Also provides an opportunity to check in on māmā and her engagement with any support service referrals identified during the Best Start Pregnancy consult.
  3. Best Start 6-week check pēpī:
    Enhanced Well Child check 6-weeks postpartum.
  4. Best Start 6-week check māmā (in development): General wellbeing check-in with māmā and follow up care. When the 6-week check for māmā is implemented it will be undertaken at the same time as the 6-week check for pēpī.

Access

Pinnacle practices who use MedTech32, MedTech Evolution and My Practice can easily install BSP using the guides outlined in resources (Best start resources).   

Pinnacle practices using indici PMS will see a rocket icon which will launch the modules.

Privacy

The tool has comprehensive security measures to ensure protection of patients according to best practice.  

  • There is a consenting question and prompt at the start of the assessment which must be discussed with the person and completed.
  • Identifiable information will be shared with a third party. Details of this included in the onboarding and training process. Details of this included in the onboarding and training process and information is included in the assessment form.  
  • Vetting process for any health providers who apply to use the Best Start Online Tool (obtained through Pinnacle).  

Note for reconciliation purposes

It is recommended for that the practice sets up Best Start pregnancy as a 'company' to enable reconciliation against payments received from National Health Coalition via Pinnacle. Please contact the NHC implementation lead (details below) if you have any issues during this process.

Who is eligible under this category?

Best Start pregnancy can be used for all pregnant women. There are incentive payments available to all health providers who complete the assessment with pregnant Māori women and/or Māori babies. 

Who is excluded from the service?

There are no exclusions to this service.

When is the service complete?

At completion of each Best Start kōwae assessment. 

Claiming guidelines

The Gen2040 project is providing incentive payments for all health providers completing the modules with pregnant Māori women and/or Māori babies. There are no incentive payments for non-Māori. 

  • Best Start Pregnancy tool: $100 incentive payment.
  • Best Start Pēpi tool: $50 incentive payment.

Important notes on incentive payments

  • Best Start Pregnancy incentive payments are made when assessments are submitted for Māori mothers or mothers pregnant with Māori babies.
  • Best Start Pēpi incentive payments are made when assessements are submitted for Māori babies.
  • Forms are able to be submitted partially completed, full payments will still be made.
  • Essential fields include demographic information, if consent was gained to complete the assessment and date of Last Menstural Period (Best Start Pregnancy).
  • Multiple assessments submitted for the same pregnancy ID within a four week period will result in one incentive payment being made.
What level of clinical notes do I need to submit?

Not applicable. 

Does the patient have to pay?

No. 

How is the service funded?

There is no cost to using the Best Start kōwae. Health providers can freely access and use the assessment.

This service is funded by the National Hauora Coalition. 

Best Start writes back to the PMS

Best Start writes a short consult note confirming that a Best Start Pregnancy Assessment has been completed.

  • The form, as completed, is saved to "Forms".
  • The formatted assessment is saved to the patient's outbox.
  • It saves the LMP and EDD and that the patient is pregnant to the PRGS screening template (Medtech32/Evolution) or the Maternity Summary (Indici).
  • Height.
  • Weight.
  • Blood pressure.
  • Recall for pertussis vaccination.
  • Recall checking on midwife engagement.
  • Recall for flu vaccination.
