Physiotherapy service: Hamilton City

Overview

This service comprises a fully funded package of care (a combination of one-on-one treatment and group education sessions) delivered by Body in Motion physiotherapy across their five Hamilton sites.

This is funded under the CPCT programme

This is a referral service only. It is not a claimable service.

Details

Which practices can claim for this service?

Any practice within the Hamilton City boundary.

Who is eligible for the service?

Patients aged over 18 who are eligible for publicly funded care in Aotearoa New Zealand AND are Māori or Pacific patients or CSC holders who have been assessed and identified as amenable to non-surgical management.

This includes but is not limited to:

  • patients frequently presenting acutely for management of osteoarthritis
  • patients known to not meet the secondary care threshold for management of osteoarthritis
  • patients on waiting list for surgery for management of osteoarthritis > 180 days
  • acute presentations for management of osteoarthritis excluding ACC cases
  • patients post intra articular corticosteroid injections.
Who is excluded from the service?

Patients funded under ACC are not eligible for this service.

Referral guidelines

This is not a claimable service; this is a free service that you can refer patients to.

Practices are required to submit an e-Referral requesting a package of care for their patient. 

Within BPAC search 'cpct' and select 'Body in Motion CPCT Physiotherapy'.

What level of clinical notes do I need to submit?

On the clinical details tab please include the joint(s) involved, duration of symptoms, treatment to date, imaging results and any other relevant information.

Does the patient have to pay?

No. There is no charge to the patients for this referral service. 

How the service is funded?

The service is funded by Te Whatu Ora as part of the CPCT service.
