Postnatal māmā hypertension follow up

This service provides funding for up to two GP/NP consults per māma for up to eight weeks post-delivery.

Which practices can claim for this service?

All Pinnacle practices in Tairāwhiti region can claim for this service.

Who is eligible for the service?

Entry into the service would be via specific recommendation for follow up from the Hauora Tairāwhiti obstetrics and gynaecology department.

Entry will be indicated in the māma’s discharge summary from maternity which will specifically note that the māma is on the ‘hypertension follow up pathway’.

Patients domiciled in Te Whatu Ora Tairāwhiti area.

Who is excluded under the service?

Patients not domiciled in Te Whatu Ora Tairāwhiti area.

Any patient not identified by Te Whatu Ora Tairāwhiti obstetrics and gynaecology department.

Please note if a review of claiming indicates that a claim has been made for a patient not identified by Te Whatu Ora Tairāwhiti then you will be required to repay the claim.

Claiming guidelines

All services are provided at no cost to the patient. If the claim cannot be funded by this service, the patient may be liable to the practice for the fees incurred.

Please make your claim via Primary Options, select Postnatal māma hypertension follow up  and then attach the appropriate invoice(s).

Price is GST inclusive.

Postnatal māma hypertension follow up

When invoicing please lodge an ‘outcome and invoice’ form and select the following invoice.

Postnatal māma hypertension consultation: $75.00

This claim can only be made twice per identified patient.

Please note if a review of claiming indicates that a claim has been made for a patient not identified by Te Whatu Ora Tairāwhiti then you will be required to repay the claim.

What level of clinical notes do I need to submit?

Practices are required to provide sufficiently detailed consultation notes to determine appropriate use of funding

It has been recommended that in addition to a good assessment and history, the full range of appropriate observations should be documented.

When is the service complete?

Once the consultation has occurred the case requires an outcome to be lodged. No payment can be made for any claims unless the outcome is completed.

Does the patient have to pay?

A patient co-payment cannot be charged.

How is the service funded?

The service is funded by Te Whatu Ora.
