Rural Accelerated Chest Pain Pathway (RACPP)

This programme allows for the management of patients with low-risk chest pain (an EDAC score lower than 16).

Which practices can claim for this service?

All Pinnacle practices in the Waikato provided they have an iSTAT equipment, and their staff have had appropriate training. (See equipment and supplies below.)

Service components

This service provides funding as a package of care for management under the RACPP over a 2-hour period regardless of outcome.

This allows for follow-up of patient’s 2-hour results and up to 115 mins of practice observation time.

Who is eligible for the service?

Patients presenting to general practice with an episode of chest pain with onset within the last 72 hours.

Patients with an EDAC score lower than 16. 

Patients who receive a clinical assessment and meet the eligibility criteria in accordance with the suggested management.

Who is excluded from the service?
  • Patients not enrolled in a Waikato general practice.
  • Out of region patients do not have funding provided under this programme.

Patients with:

  • ST segment elevation myocardial infarction present on ECG
  • an EDAC score 16 or above
  • haemodynamic instability
  • active chest pain that is suspected to be of cardiac origin
  • chest pain clearly due to non-coronary pathology
  • suspected PE, pancreatitis, AAA or STEMI.

Any patient who requires transfer to hospital regardless of a negative troponin, due to other medical conditions, or need for other investigations.

Claiming guidelines

All services are provided at no cost to the patient. If the claim cannot be funded by this service, the patient may be liable to the practice for the fees incurred. Please note that if a review of claiming indicates that a claim has been paid for a patient who was not eligible for this service, then you will be required to repay the claim.

This price is GST inclusive.

RACPP: $342

This package of care is for a low-risk patient who has completed the RACPP over a 2-hour period regardless of outcome.

This invoice includes an allocation for staff time as well as consumables, it cannot be claimed along with any other claims.

This allows for follow-up of patient’s 2-hour results and up to 115 mins of practice observation time.

These claims are limited to one per day per patient per provider.

What level of clinical notes do I need to submit?

Practices are required to provide sufficiently detailed consultation notes to determine appropriate use of funding.

It has been recommended that in addition to a good assessment and history, the full range of appropriate observations should be documented, especially where the diagnosis is undetermined. It is important to state the time of consultations and interactions with the patient.

Does the patient have to pay?

No. A patient co-payment cannot be charged.

Equipment and consumables

iSTAT point of care (POC) testing equipment and consumables can be purchased from Abbott Pharmaceuticals.

For repairs or servicing please contact Abbott directly. There is the option to obtain a 12-month service contract to cover any repairs that may be needed, and a loan device is also arranged with this option.

Pathlab: Changes to B-type Natriuretic Peptide (BNP) testing

Pathlab has reported BNP levels using the Triage Method and using EDTA plasma as the preferred sample type. The current reference interval is 0-100 ng/L. From 23 December 2024, the current BNP method will be replaced by NT-proBNP. This method is more specific than BNP, as NT-proBNP has a longer half-life and is less influenced by factors like obesity or kidney function.
