New pathway for children with glue ear

A new Advanced Audiology Practitioner (AAP) clinic has been operating at Taranaki Base Hospital since January 2026, introducing a new model of care for children referred with suspected otitis media with effusion (OME/glue ear).

Children referred to ENT may now be assessed initially by advanced audiologists under ENT oversight, with each assessment covering clinical history, hearing assessment, otoscopy, and management planning with whānau.

Early outcomes show that around 25 per cent of children are listed directly for grommet surgery, while a further 25 per cent are discharged back to primary care with advice and no surgical intervention required.

The referral pathway for primary care remains unchanged — continue to refer suspected OME to ENT as usual. Some children will be returned to primary care following assessment, with monitoring plans, advice on hearing and speech/language concerns, and clear re-referral guidance. ENT input is focused on children most likely to benefit from surgical or specialist intervention, supporting more targeted use of specialist resources.

This audiology-led model provides tamariki with faster access to assessment, reduced unnecessary specialist appointments, and quicker progression to surgery where appropriate, ensuring children receive the right care at the right time.
