Practices have been able to use the Annual Statement of GP Reasonable Fee Increases (Annual Statement) since it was first released in 2006.
The Annual Statement for each year covers the period 1 July to 30 June.
The Annual Statement applies only to the fees set by Interim and Low Cost Access practices. Very Low Cost Access Funded (VLCA) practices receive extra funding for their enrolled patient base and in return have to set their fees below or at a threshold set by Te Whatu Ora - Health New Zealand.

Each Interim or Access practice (non-VLCA) with be sent their own annual statement Co-payment Adjustment Templates - Option A and Option B along with the Annual Statement Template Guide and the Interim or Access Practice Fee Change Notification Form.
The Annual Statement Template Guide has been created to support you in the use of the template and a download copy is also available at the bottom of this page.
The Interim or Access Practice Fee Change Notification Form is to notify us of the changes to your Non-CSC and/or CSC fees including the date that you intend to change these fees. Only Non-CSC fee changes need to follow the annual statement process as above.

If you decide to change your fees, please complete the VLCA practice fee change notification form 2023/24 (below) and email it to the practice systems support team.
The PHO agreement requires us to notify Te Whatu Ora what fees Very Low Cost Access (VLCA) funded practices are charging their enrolled patients. Please also remember to update your practice Healthpoint listing with this information to ensure that our website displays your practice’s correct fees.
